Getting started as a CaptionCall Provider is easy – just click on the ‘Register Now’ button shown below and submit your contact information.  Your local CaptionCall Specialist will contact you to schedule a brief consultation and provide you with brochures and certification/order forms.

Free with Professional Certification

For your residents to qualify for the free CaptionCall phone and service, they must obtain certification from a healthcare or hearing-care professional, certifying that they have hearing loss and need CaptionCall to communicate effectively by phone.  Once you’ve registered as a CaptionCall Provider, you will receive certification forms pre-populated with your information and a unique promo code to use when ordering a phone for someone.  To see a sample of a generic certification form, click on this ‘Certification Form’ button.

Anyone who has trouble using the telephone due to hearing loss is a good candidate for CaptionCall.  There is no predetermined decibel threshold to qualify.  Your residents must simply have a hearing loss that, in the opinion of a hearing or healthcare provider, necessitates the use of CaptionCall to communicate effectively by phone.

System Requirements

  • CaptionCall Phone – the qualified individual will need a standard home phone line and an Internet connection. 
  • CaptionCall Mobile – they will need a CaptionCall account, an Apple® iPad2 or later, and a Wi-Fi or cellular Internet connection.

How to Order CaptionCall

Help your clients order a free CaptionCall phone in two easy steps:
  1. Visit CaptionCall.com, click on the ‘Request Phone’ button, and help them complete and submit the online order form.
  2. Provide your resident with a Professional Certification Form to take to their doctor or hearing specialist to have it signed, and then fax to 1-888-531-1906 or 1-888-778-5838 or email to certification@captioncall.com
When we receive the online order form and completed certification form, a CaptionCall representative will contact your referred resident to schedule the delivery and installation of a free CaptionCall phone.
